I think it is caused by stress concentration, you can try to subdivide this element, and then apply the vertical load on several top nodes in the central zone.

You can simulate the pre-stresses reinforcing steel in the material level by using steel02.
uniaxialMaterial Steel02 $matTag $Fy $E $b $R0 $cR1 $cR2 <$a1 $a2 $a3 $a4 $sigInit>
define the parameter $siglnit to set the initial stress.

You can use it to model the hysteretic loop of element (force-disp relationship),or section (such as moment-curvature relationship), or material (stress-strain relationship).

If we want to get the length of the plastic hinge as accurate as possible, maybe we should subdivide the specimen into several elements or increase the number of integration points. I think this quite relys on the distribution of the integration points and the discretization of the specimen.

I have also done some research on this model, I think it is not due to the accumulation stuff. This model really works well in all static analysis, only not fit for dynamic analysis.

Firstly, rigid diaphragm can only be utilized in 3D model.
